Effects of curcumin supplementation on vitamin D levels in women with premenstrual syndrome and dysmenorrhea: a randomized controlled study

3-month, randomized, triple-blind, placebo-controlled trial; single females aged 18–24 years with a history of moderate-to-severe PMS and dysmenorrhea; recruited from dormitories of four universities in Birjand, Iran; 76 enrolled (38 per group); 73 completed (36 in CUR, 37 in placebo).
Intervention
Curcuminoids (CUR) 500 mg + 5 mg piperine per capsule; one capsule daily for 10 days per cycle (7 days before to 3 days after onset of menstruation) for 3 consecutive cycles. Placebo: lactose 500 mg + 5 mg piperine per capsule; same regimen.
Results
CUR significantly increased serum vitamin D within-group from 12.8 ng/mL (IQR 7.0-24.6) to 16.2 ng/mL (IQR 6.4-28.8) (P=0.045). Placebo changed from 18.6 ng/mL (IQR 2.2-26.8) to 21.3 ng/mL (IQR 5.2-27.1) (P=0.17). End-of-trial vitamin D improvement occurred in 25% of CUR participants vs 18% in placebo (between-group p=0.71; CUR within-group p=0.039). CUR reduced AST (P=0.040) and direct bilirubin (P=0.021); placebo showed no significant changes. Fasting glucose did not differ. Conclusion: Three-cycle curcumin supplementation may modestly improve vitamin D status and certain liver function markers in young women with PMS and dysmenorrhea, without impacting glucose levels; findings warrant confirmation in larger, longer studies.
Limitations
Small sample size; short duration (3 cycles); baseline vitamin D levels differed between groups; three withdrawals due to rash in CUR group; limited generalizability beyond young Iranian women with PMS/dysmenorrhea.
